Traumatic Longitudinal Atlanto-occipital Distraction Injuries

چکیده
Traumatic atlanto-occipital dislocation with survival is possible and , in fact may be relatively more common than once thought. The spectrum of neurologic manifestations is broader than previously described and does not necessarily end in death or tragic neurologic deficit. Radiographic diagnosis of this injury may be d ifficult , particularly in the longitudinal distraction-dislocation type. Although several methods have been proposed to evaluate the atlanto-occipital relationship, none of these is infallible in the recognition of distraction injury in children. Immobilization rather than skeletal traction provides sufficient immediate stabi lization when the dislocation at the atlanto-occipital junction is of the longitudinal distraction type. Three cases are reported : in one, death occurred early; in the second, recovery was partial, but sudden death occurred 2 years later; the third child recovered fully.

Traumatic Atlanto-Occipital Dislocation Presenting With Dysphagia as the Chief Complaint: A Case Report
We report a patient with traumatic atlanto-occipital dislocation who presented with dysphagia as the chief complaint. A 59-year-old man complained of swallowing difficulty for 2 months after trauma to the neck. On physical examination, there was atrophy of the right sternocleidomastoid and upper trapezius muscles, and the tongue was deviated to the right. In a videofluoroscopic swallowing study...

We present four patients who had sustained a traumatic dislocation of the atlanto-occipital joint. The diagnosis was initially missed in two patients. One patient, who was neurologically intact, was treated non-operatively. The remaining three recovered neurologically after an occipitocervical fusion. Early recognition of the injury, especially in multiply-injured patients with head injuries, a...
متن کاملAtlanto-occipital dislocation: Case report and discussion
Traumatic atlanto-occipital dislocation is an uncommon injury that frequently results in either a fatal outcome or severe neurologic deficit. This diagnosis must be considered for any patients who may have had cervical spine damage after high trauma, even in the absence of neurologic signs, as there have been reports of cases without neurologic impairment. In addition to radiographic examinatio...

The unique anatomy of the upper cervical spine and the typical mechanisms of injury yield a predictable variety of injury patterns. Traumatic ligamentous injuries of the atlanto-occipital joint and transverse atlantal ligament are relatively uncommon, have a poor prognosis for healing, and often respond best to surgical stabilization. Bony injuries, including occipital condyle fractures, atlas ...

DIAGNOSTIC STANDARDS There is insufficient evidence to support diagnostic standards. GUIDELINES There is insufficient evidence to support diagnostic guidelines. OPTIONS A lateral cervical x-ray is recommended for the diagnosis of atlanto-occipital dislocation. If a radiological method for measurement is used, the basion-axial interval-basion-dental interval method is recommended. The pre...
